This will be a fixed term post up till May 2020, with a possibility of extension to September 2020.
Salary – £21,277
Hours – 35 (Plus 5 hours paid lunch)

Who we are looking for?
We are looking for a keyworker to work as part of a new multi-disciplinary team to provide targeted support and service for people with a diverse range of complex needs. The Key Worker will work intensively with adults aged 18 plus who are experiencing a range of complex issues that act as barriers to reaching their full potential. The role will provide time limited assertive support that enables individuals to address the immediate issued affecting them, whilst building upon their individual assets and resilience.
Duties and responsibilities include but are not limited to:
1. Carry out a proportionate assessment of the person or people accessing the service including any risk
2. Work with individuals/ families referred to the service in order to assist them to articulate their desired outcome and provide a time limited assertive support service to enable them to achieve those outcomes
3. Effectively respond to crises through developing innovative solutions to intractable issues and be a key liaison to statutory services and other key agencies working with, or known to, the individuals accessing the service
4. Adopt a range of life coaching tools and techniques to help people gain greater self awareness and make positive changes to their lives
A DBS check will be required for this role.
