New Wellbeing and Self-Care service from Stockport GPs

The Wellbeing and Self-Care service is a new service offered by GP’s throughout Stockport who are members of Viaduct Care

Self Care Coordinators are in post to offer one to one support for patients who are at risk from long term conditions, social isolation and low emotional wellbeing.

Services offered include support with finances, activities and low-level mental health.
